Tips for Completing the Application

The Cook County Arts Nonprofit Relief Grant application is straightforward. It should take around 20 minutes to complete (especially if you have your documentation ready to go!). The grant application is available here.

  1. Gather all your required documents and information first! 

Before you start your application, have all your information and documentation ready, either to upload or fill in. You are NOT able to save and come back later to complete and submit! 

Make sure you have the following ready as these are all required - listed in order that they appear in the application (Question # in parentheses):

  • Federal Employer Identification Number (EIN) (#4)

  • Date of Incorporation/Registered Effective Date (they are identical) (#11)

  • Organization’s operating budget for 2019 and for the most recent fiscal year.(#15 & #16)

  • Organizational 990 tax return for 2019 (#17)

  • Organizations established between January 1, 2019, and March 11, 2020, should upload their first 990 filing. IRS form 990, 990-EZ, and 990-N (e-postcard) accepted.

  • If you file the 990-N (e-postcard), please upload your AG990-IL Charitable Organization Annual Report (link shows a sample blank form, for your reference).

  • Bank statement from a current, active bank account where the funds will be deposited if your organization receives a grant. (#17)

  • The following information must be clear:

  • Name on the bank account - it must match the applicant name or other listed holders on 990 applying for grant.

  • Last 4 digits of Account Number. If selected for an award, this information will be used to verify the bank account you link for payment.

  • Government-issued photo ID of Officer or Trustee listed on 990 or AG990-IL Charitable Organization Annual Report (#17)

  • You will be asked to submit a photocopy of the front and back of the ID.

  • Optional work sample: have a work sample ready that is either in JPG or PDF format, or have a URL or web link to websites, documents, video or audio recordings, or images. (#17)

  • Required demographic information - you will need to fill in the follow data:

  • Total number of W2 employees (#21)

  • Average total number of 1099 contractors working for your organization in a given year (#22)

  • Population types your organization serves - they will be listed and you can select all that apply. (#23)

  • Percentage of organization’s staff and executive leadership are:

  • Women or LGBTQ+ (#24)

  • Black, Indigenous or other People of Color (BIPOC) (#25)

  • Whether your organization tracks the demographic makeup of individuals directly served (Y/N) (#26)

  • Percentage of individuals served by your organization in the last year who are

  • Women or LGBTQ+ (#27)

  • Black, Indigenous or other People of Color (BIPOC) (#28)

  • Banking information - the banking information should match the bank statement or check provided in Document Uploads: (#33-35)

  • Bank Name

  • Bank Account Holder Name

  • Last 4 Digits of Bank Account Number

  1. Contact information for the person filling out the application and the back up person.

In addition to the required documentation, you will also be required to provide:

  • Email address for person filling out the application (#36)

  • First and last name, title, and phone number for person filling out the application (#38-41)

  • First and last name, phone number, and email address for a backup person to contact regarding the application. (#42-45)

  1. Receive a copy of your submitted application.

We strongly encourage applicants to select the box next to “Send me a copy of my responses” at the bottom of the application, just above the blue “Submit” button. You will be prompted to enter an email address where the copy of the application will be sent.
